PACE Primary Care Physician

Seeking a PACE Primary Care Physician to provide comprehensive geriatric care in Lafayette, CO, Monday-Friday, 8am-5pm, with on-call responsibilities around 5 days per month, including evenings and weekends.Deliver diagnosis, treatment, and management for PACE participants and hospice patients.Participate in interdisciplinary team meetings to develop and update care plans.Provide urgent visits in clinics or homes during work hours and on call as needed.Ensure safety and adherence to best practices in all care activities.Qualifications include 2+ years primary care experience, unencumbered Colorado license (MD/DO), DEA/control license, and experience with elderly patients preferred.
