RN Hospice Case Manager - Home Hospice Burlington & Walworth, WIDay Shift | Monday - Friday This position supports a Home Hospice program providing compassionate end-of-life care across the community. Care is delivered in multiple settings including: Patient homes Assisted Living Facilities (ALF) Skilled Nursing Facilities (SNF) Group homes Independent living communities Two hospitals for General Inpatient (GIP) hospice care The hospice program supports approximately 105 patients daily . Position Overview We are seeking an experienced RN Hospice Case Manager (RNCM) to manage and coordinate care for hospice patients. The RNCM will provide clinical oversight, patient education, and coordination with interdisciplinary teams to ensure comfort and quality of life for end-stage patients. Travelers with hospice or home health experience are preferred . Patient Population End-stage hospice patients Care provided in homes and facility-based settings Focus on comfort care, symptom management, and family support Staffing & Ratios RN Case Managers: 7 Caseload: 14-16 patients per RN CNAs: 6 LPNs: Not utilized in this unit Requirements Experience Minimum 2 years of RN experience Hospice or Home Health experience preferred First-time travelers may be considered but experience is preferred Certifications / Licensure Active RN License BLS Certification Valid Driver's License Auto Insurance (required for home visits) Technology & Equipment EMR: Epic & Rover IV Pump Medication dispensing systems Patient safety monitors (chair and bed alarms) Home health nursing bag & trunk supplies Floating Travel within the community to patient homes and care facilities Orientation 1 week orientation provided Schedule Shift: 8:00 AM - 4:30 PM Days: Monday - Friday Weekend Rotation: NoneOn-Call: NoneHoliday Requirement: None Time Off Requests: Approved through manager Scrubs Blue or Black Parking Free parking available at Burlington and Walworth offices Unit Culture This hospice team emphasizes: Strong teamwork Collaborative support Compassionate patient-centered care Ideal Candidate: A compassionate RN who enjoys building relationships with patients and families while providing high-quality hospice care in a community setting.
