CCC Case Management SupervisorAs the Case Management Supervisor, you will play a key role in supporting the daily operations and clinical oversight of CRC’s Case Management Program. This position offers the opportunity to lead frontline staff, support high-quality service delivery, and make a meaningful impact on the lives of individuals with brain injuries and complex needs. We strive to support our clients in leading meaningful lives after their injuries.Job Duties and Responsibilities:In partnership with the Program Manager, the Case Management Supervisor is responsible for supporting the day-to-day operations, coordination, and staff supervision within CRC’s Case Management Program, including services provided under the Massachusetts Statewide Head Injury Program (SHIP).Key ResponsibilitiesProgram Coordination & SHIP SupportSupport coordination and implementation of services under the Massachusetts Statewide Head Injury Program (SHIP), administered by MassAbility (MBY)Triage time-sensitive communication from SHIP Regional Supervisors and Coordinators related to contracted clientsAssist in resolving service delivery issues and escalating concerns as appropriateSupport daily coordination of client services to ensure timely and effective careStaff Supervision & SupportProvide direct supervision of Case Management staff, including regular supervision meetings and documentation of discussions and action itemsSupport staff in decision-making, documentation, and complex client situationsAssist with onboarding, training, and mentorship of new Case Management staff in collaboration with the Program ManagerSupport staff in completing incident reports and maintaining compliance with program expectationsCo-facilitate staff meetings and team communication as neededProgram OperationsOrganize and facilitate “Client Rounds” to support team collaboration and care planningAssist with maintaining program organization, workflow, and service delivery standardsProvide coverage and act as point of contact when the Program Manager is unavailable, communicating updates to leadership as neededDirect ServiceMaintain an active caseload (approximately 18–22 hours per week), providing comprehensive case management services, including:Assessment and service planningCoordination of medical, behavioral health, and community-based supportsAssistance with benefits, housing, and resource navigationCrisis intervention and mandated reporting as requiredCo-facilitate 1–2 wellness groups per weekJob Relationships:Reports to and receives supervision from the Program ManagerWorks collaboratively with the General Manager, Director of Rehabilitation, and administrative staff as neededServes as point of contact in the absence of the Program ManagerQualifications:Bachelor’s degree required in a related field (e.g., Social Work, Psychology, Mental Health Counseling, Rehabilitation Counseling, Occupational Therapy, or similar); advanced degree preferred. An equivalent combination of education and experience will be considered, including candidates actively pursuing a relevant degree with a minimum of 5 years of professional experience working with individuals with brain injuries.Minimum of 4 years of professional experience in community-based case management, including experience working with individuals with brain injuriesStrong knowledge of Massachusetts community resources and entitlement systemsExperience supporting or mentoring staff preferredCore CompetenciesStrong communication, interpersonal, and leadership skills Ability to manage competing priorities and respond to changing needsSound judgment and problem-solving in complex client situationsStrong organizational skills and attention to detailAbility to work both independently and collaborativelyWork EnvironmentThis position includes a combination of remote, in-office, and community-based work. Regular travel is required for client visits, team meetings, and program needs.
