Your recruiter will provide the most up-to-date details during the hiring process._**How you'll make an impact in this role**+ Develop, implement, and regularly reassess individualized, patient-centered plans of care in collaboration with patients and families.+ Deliver evidence-based interventions across complex diagnoses, including neurological, oncological, orthopedic, cardiopulmonary, and medically complex conditions.+ Collaborate with the interdisciplinary team to coordinate care, optimize outcomes, and support safe, effective discharge planning.+ Utilize innovative rehabilitation technologies and best practices to maximize functional recovery and independence.**What minimum requirements you'll need**Licensure / Certification / Registration:+ BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.+ Physical Therapy (PT) license credentialed from the Illinois Department of Financial and Professional Regulation prior to hire date or job transfer date required. A valid license pending letter will be accepted until license is issued.Education:+ Bachelor's degree required.
