Licensed Practical Nurse- Primary Care Women's Clinic

This Licensed Practical Nurse (LPN) position is located within the Omaha VA Medical Center, Women's Health Clinic. As part of a Patient Aligned Care Team, the LPN delivers Veteran-centered, care to women with complex health needs. In this role, you will manage patient panels, support clinic operations, coordinate care across services, and perform clinical tasks such as intake, vitals, lab collection, medication administration, and assisting with procedures using trauma-informed care.Total Rewards of a Allied Health ProfessionalMajor duties include but are not limited to:Provide direct, Veteran-centered nursing care in the Women's Health Clinic, addressing both physical and mental health needs.Perform clinical tasks including patient intake, vital signs, lab specimen collection, and assisting with procedures.Administer medications, immunization, and treatments.Manage assigned patient panels, clinic schedules, and daily workflow.Perform support duties for diagnostic procedures including preparing patient, assisting in the diagnostic examination and monitoring patient condition while using trauma-informed principles and maintaining sensitivity to the unique needs of women Veterans.Coordinate care with specialty, surgical, ancillary, and community-based services to ensure continuity and quality of care.Serve as a liaison and advocate for patients, families, and interdisciplinary team members to support optimal outcomes.Collaborate with the Patient Aligned Care Team (PACT) to assess, plan, implement, and evaluate individualized care plans.Participate in population health management and proactive outreach for assigned patient panels.Contribute to quality improvement initiatives and unit-level performance improvement projects by identifying areas of improvement, collecting patient data, monitoring and reporting outcomes, and participating in committees or workgroups.Other duties as assigned.Work Schedule: Full time, Monday - Friday; 8 a.m. - 4:30 p.m.Physical requirements: This position requires some degree of fitness, strength, and agility. Duties require extensive periods of standing and walking. Working with patients requires regular and recurring bending, lifting, stooping, stretching, pushing, pulling, lifting arm overhead, and similar activities. You will be asked to participate in a pre-employment examination or evaluation as part of the pre-employment process for this position. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).
