The Renal Dietitian is responsible for providing nutrition education, counseling and interventions as appropriate to End Stage Renal Disease (ESRD) patients and authorized caregivers in clinical settings to maximize the patient's nutritional well-being.What Will You Do?Nutrition Assessment & Care PlanningConducts initial and ongoing nutrition assessments, develops individualized careplans, and updates them in collaboration with the interdisciplinary team (IDT).Medical Nutrition Therapy & MonitoringDesigns and adjusts nutrition interventions based on lab results, dialysis adequacy,and patient goals. Monitors progress and documents monthly.Patient Education & CounselingProvides tailored nutrition education using appropriate materials, considering eachpatient's medical, social, and economic background.Team Collaboration & Facility SupportParticipates in care conferences and quality improvement meetings. The work environment is typical of a healthcare facility, featuring controlled temperatures and moderate noise levels, with potential exposure to infectious or contagious materials.
