Transitional Care Coordinator/Telehealth Nurse

Duties & Responsibilities Coordinate seamless transitions of care between providers (physicians, acute care, skilled care, and home care), ensuring patient and family needs are met with excellent customer service Deliver clinical education prior to and on the day of transfer to support safe and informed transitions Oversee telehealth program operations, including telephone and monitor setup, follow-up, and reporting findings to the primary nurse Support nurse managers by taking physician orders and responding to calls from staff and patients Escalate urgent issues to the operations manager as appropriate Qualifications AnMed will accept applications from Graduate Nurses that meet SC LLR criteria. Licensed Registered Nurse in South Carolina. Minimum of three (3) years clinical experience and two (2) years home care experience required ADN candidates must be enrolled in BSN program within 18 months of hire date. BSN is required within 4 years from date of hire. Successful completion of hospital provided BLS course within 90 days from date of hire.
