Guaranteed Hours – 36hr
Pay rate - $23/hr. to $26/hr.
Shift – 6:45pm -7:15am

Job Summary: The Certified Nursing Assistant (CNA) provides direct patient care and assists with activities of daily living (ADLs) for patients in a nursing and rehabilitation facility. The CNA works under the supervision of registered nurses (RNs) and licensed practical nurses (LPNs) to ensure the comfort, safety, and well-being of patients undergoing rehabilitation and nursing care.
Key Responsibilities:
Patient Care:
Assist patients with activities of daily living, including bathing, dressing, grooming, toileting, and mobility.
Provide support with feeding and hydration, including the use of adaptive equipment as needed.
Monitor and record vital signs such as blood pressure, temperature, pulse, and respiration.
Assist patients with exercises and physical therapy routines as directed by the rehabilitation team.
Support and Communication:
Offer emotional support and companionship to patients, addressing their concerns and providing comfort.
Communicate effectively with patients, families, and the healthcare team regarding patient needs and progress.
Report any changes in patient conditions or behaviors to the nursing staff promptly.
Safety and Hygiene:
Maintain a clean and safe environment for patients, including proper handling and disposal of medical waste and maintaining infection control practices.
Ensure patient safety by assisting with fall prevention and using proper lifting and transfer techniques.
Documentation:
Accurately document patient care activities, observations, and any changes in patient conditions in electronic health records or paper charts as required.
Follow facility protocols for documenting and reporting patient information.
Rehabilitation Support:
Assist with rehabilitation activities, including ambulation, range of motion exercises, and use of rehabilitation equipment.
Collaborate with physical, occupational, and speech therapists to support patient progress and recovery.
Team Collaboration:
Work closely with the nursing team to implement patient care plans and provide comprehensive care.
Participate in care team meetings and contribute to the development and evaluation of care plans.
Qualifications:
Certification: Must hold a valid Certified Nursing Assistant (CNA) certification from an accredited state or national certification program.
Education: High school diploma or equivalent required.
Experience: Prior experience in a healthcare or rehabilitation setting is preferred but not required.
Skills:
Strong communication and interpersonal skills.
Ability to work effectively as part of a healthcare team.
Basic knowledge of medical terminology and healthcare procedures.
Proficiency in documenting patient care and using electronic health records.
Physical stamina and the ability to perform tasks involving lifting, bending, and standing for extended periods.
Physical Requirements:
Ability to lift and transfer patients, with or without assistance.
Capability to stand, walk, and bend frequently throughout the shift.
Visual and auditory acuity to monitor and assess patient conditions.
Working Conditions:
May include working in a variety of environments within the facility, including patient rooms and common areas.
Potential exposure to infectious diseases and hazardous materials, with appropriate safety precautions provided.
