ECM Lead Care Coordinator (Case Manager)

Role Description This is a full-time role for an ECM Lead Care Coordinator (Case Manager), based in Garden Grove, CA. The Lead Care Coordinator will focus on providing care coordination and case management services, ensuring that clients receive the support and resources needed to improve their health and well-being. Responsibilities include client assessment, working with PCPs, hospitals, medical providers, connecting clients with services such as home care or other community resources, maintaining comprehensive case records, and collaborating with partners to optimize client outcomes. Additionally, the role involves providing exceptional communication and customer service to clients and stakeholders. Qualifications Proven experience in Medical, Care Coordination and Case ManagementStrong Communication and Customer Service skillsKnowledge or experience in health care navigation for individual with chronic health conditionsAbility to manage client interactions with empathy, cultural competence, and professionalismDetail-oriented with excellent organizational and time-management skillsProficiency with data entry and maintaining accurate and timely case recordsBachelor’s degree in social work, healthcare, or a related fieldExperience in the nonprofit or community services sector is highly desirable
