Transition Coordinator
Job Summary
We are seeking a dedicated and highly organized Transition Coordinator to join our healthcare team. In this vital role, you will oversee patient transitions across various care settings, ensuring seamless coordination of discharge planning, case management, and utilization review processes. Responsibilities
Coordinate patient discharges by collaborating with multidisciplinary teams including nursing, social work, case management, and physicians to develop comprehensive transition plans.
Facilitate communication between hospitals, outpatient providers, hospice care agencies, and community resources to ensure smooth patient transfers.
Conduct thorough intake assessments and evaluate patient needs for appropriate placement or follow-up care.
Manage utilization review and utilization management activities to ensure appropriate level of care and resource allocation.
Oversee documentation within electronic health record (EHR) systems while maintaining strict adherence to HIPAA regulations.
Assist in discharge planning by reviewing medical records, coding (including CPT, ICD-9, ICD-10), and verifying insurance authorizations.
Support case management efforts by identifying barriers to discharge and implementing strategies to overcome them.
Participate in multidisciplinary team meetings to review patient progress and coordinate ongoing care plans.
Monitor compliance with hospital policies related to behavioral health, hospice care, pediatrics, trauma centers (Level I & II), ICU, PICU, OR settings, and emergency medicine.
Contribute to quality improvement initiatives focused on patient service excellence and efficient hospital operations.
Requirements
Proven experience in hospital settings with a strong background in acute care environments such as ICU, PICU, trauma centers, or emergency departments.
Critical care experience is highly preferred; familiarity with Level I & II trauma center protocols is advantageous.
Knowledge of medical terminology, CPT coding, ICD coding (ICD-9 & ICD-10), and discharge planning procedures.
Experience working within EMR/EHR systems with a solid understanding of documentation standards.
Strong understanding of HIPAA regulations and confidentiality protocols related to patient information.
Background in social work or behavioral health services is desirable.
Excellent organizational skills with the ability to manage multiple priorities efficiently.
Effective communication skills for collaboration across diverse healthcare teams.
Familiarity with managed care processes and utilization review practices.
Ability to adapt quickly within dynamic hospital environments such as hospital medicine units or specialty departments.
