Job Description Job Opportunity: Family Practice or Internal Medicine PhysicianOur organization is currently looking for a full-time, Board-Certified or Board-Eligible physician specializing in Family Practice or Internal Medicine to join our outpatient clinic located in Fairview, Oregon. This role will serve as the Primary Care Clinical Practice Manager Physician. As part of the Portland VA Medical Center network, you will deliver services within a comprehensive system of outpatient clinics and hospitals throughout Oregon.The primary care provider will operate as a highly skilled, independent practitioner, managing patient care while collaborating with an interdisciplinary team. You will have access to cutting-edge medical technologies and state-of-the-art facilities.Key Responsibilities:Lead and manage patient care within the PACT (Patient Aligned Care Team) model.Oversee a panel of approximately 900 patients, addressing both acute and chronic health conditions.Work alongside a team that includes a Primary Care Provider (PCP), Registered Nurse (RN), Licensed Practical Nurse (LPN), and Medical Support Assistant.Provide comprehensive care for various conditions such as diabetes, hypertension, chronic obstructive pulmonary disease, and more.Qualifications:Must be a U.S. Citizen.Possess a current, unrestricted license to practice medicine or surgery in the United States.Board Certification in Family Practice or Internal Medicine is required.Demonstrated leadership and administrative skills.A strong commitment to staff education, professional development, and positive labor relations.Work Schedule:This position operates Monday through Friday, with no weekend or holiday shifts required.
