Salt Lake City (SLC) VA Health Care Systems has three RN's, Community Care Coordinator positions open. Option to work either or both locations is available in South Jordan, UT. or SLC, UT. Responsible & accountable for all elements of the nursing process when providing & /or supervising direct patient care. Assesses, plans, implements, & evaluates the effectiveness of the Veteran's care. Serves to support delivery of high quality, timely care to the Veteran receiving services in the community.The Community Care (CC) Nurse is a licensed professional registered nurse assigned to coordinate care for Veterans requesting or referred for community care.RN is responsible for evaluating care requested as a delegate of authority (DOA), as established by Chief of Staff, and providing the clinical guidance and expertise for the coordination of community care.This includes assessment, planning, implementing and evaluating care.RN assigned to Community Care has evidence of self-direction and application of care management and/or utilization management, with a broad knowledge of clinical treatment modalities.A basic understanding of the organization and administration of multifaceted health care delivery systems, specialty and subspecialty care, educational principles, data base management, and knowledge of accreditation standards.Responsibility for the coordination of care focused on patient education throughout the continuum of careResponsibility for the coordination of care focused on Self-management and customer satisfaction throughout the continuum of careFollows procedures per established policies and guidelines.Influences care outcomes by collaborating with members of the interdisciplinary team.Core elements of performance include knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs.Preferred Experience:Care Navigation & consult: Skilled in assisting Veterans through care coordination processes; both VA and community-based; ensuring adherence to VHA standards and clinical decision-making guidelines.Community care coordination: Demonstrated ability to manage and triage Community Care consults for clinical appropriateness, including Request for Services (RFS), Patient-centered care coordination.Strong critical thinking & clinical judgment: Proven ability to apply nursing knowledge and systems theory to assess, plan, coordinate, and evaluate care, ensuring high-quality, timely outcomes.Interdisciplinary collaboration: Experience working with multidisciplinary teams (e.g., integrated veteran care, RCI, medical center staff, community providers) to facilitate efficient, quality-driven patient care.Health information systems proficiency: Proficient in computer systems and technology; adapt quickly to new software and tools such as VA specific systems and Microsoft Office Suite.VA offers a comprehensive total rewards package: VA Nurse Total RewardsPay: Competitive salary, regular salary increases, potential for performance awardsPaid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VAInsurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)Licensure: 1 full and unrestricted license from any US State or territoryWork Schedule: 8:00am to 4:30pmCompressed/Flexible: Not availableTelework: Yes-This position is authorized for telework-situational (ad-hoc) AND has not been authorized as remote.Virtual: This is not a virtual position.Relocation/Recruitment Incentives: Not availableEducation Debt Reduction Program (EDRP)- Not availablePermanent Change of Station (PCS): Not AuthorizedPCS Appraised Value Offer (AVO): Not Authorized
