Our medical group features board-certified professionals across more than 50 specialties.We are currently seeking applications from Board Certified or Eligible Internists, Family Practitioners, Geriatricians, or Hospitalists to join our expanding medical team within the Continuum Complexivist department. This position will primarily focus on the Portland, OR metropolitan area. The ideal candidate will be committed to delivering high-quality, comprehensive care. You will be part of a compassionate and integrated team of physicians, along with supportive clinical and administrative staff, working within our core Skilled Nursing Facilities (SNF) and virtually through our Medicare Home Health Physician program.What You Can Expect:This team consists of nurses, therapists, and social workers who provide post-hospital care in the SNF environment. We prioritize smooth transitions of care from hospitals to skilled nursing facilities and from SNFs to home or other care settings. Our approach is team-oriented, typically involving one to two SNF physicians (MD/NP) alongside a Kaiser Care Coordinator. You will manage a diverse range of medical complexities, from straightforward elective orthopedic surgery cases to patients with multiple complex medical issues at each facility.Our departmental objectives include reducing emergency room visits and hospital readmissions, as well as facilitating transitions to palliative and hospice care when necessary. You will enjoy a primarily weekday, daytime schedule. Most of your work will take place at one of our nine SNF locations, but you will also support a virtual care model that includes early, daytime, and evening shifts once a week. Comfort in navigating the Portland metro area is essential. There may be some flexibility, allowing for a focus on one primary SNF location.During virtual shifts (conducted from home), you will assist our home health field staff in members' residences and provide consultations to determine patient eligibility. You will also collaborate with our "Transition Nurse Team" for escalations involving patients transitioning from recent Hospitalist or SNF discharges. Virtual shifts will constitute approximately 10-20 percent of your workload. Familiarity with digital technologies (such as AI platforms, telehealth, and digital health) is preferred. If you lack direct experience, you should demonstrate flexibility and adaptability to integrate these technologies into your practice as we advance our virtual care model.
