Registered Nurse (RN) - HospicePosition SummaryThe Registered Nurse (RN) provides nursing care to hospice patients in their place of residence and coordinates care with the Interdisciplinary Team (IDT), physicians, patients, families, and referral sources. The RN is responsible for assessing patient needs, implementing the plan of care, and ensuring quality end-of-life care.ResponsibilitiesPerform initial and ongoing patient assessmentsDevelop, implement, and update individualized plans of careCoordinate care with the IDT, physicians, patients, and familiesProvide patient and caregiver education regarding disease process, medications, end-of-life care, and safetyDetermine visit frequency based on patient needs and acuitySupervise Hospice Aides and LVN/LPN staffDocument clinical findings, interventions, and patient progress accuratelyReport changes in patient condition to physicians and team membersPractice infection control and standard precautionsParticipate in QAPI and hospice in-service education programsProvide on-call services as assignedPromote hospice philosophy and quality patient careQualificationsGraduate of an accredited Diploma, Associate, or Bachelor’s School of NursingCurrent RN license in the state of practice and/or compact license eligibilityMust have hospice experienceUnderstanding of hospice regulatory requirements, documentation standards, and compliance guidelinesStrong clinical, communication, and interpersonal skillsKnowledge of end-of-life and palliative careAbility to read, write, and communicate effectively in EnglishRequirementsCurrent driver’s licenseReliable transportationValid auto liability insuranceWorking ConditionsTravel to patient homes requiredPossible exposure to blood, bodily fluids, and infectious diseasesFlexible Schedule, Local Travel, And On-call Responsibilities RequiredAbility to stand, walk, lift up to 50 pounds, and assist with patient transfersNOTE: Per-Visit Rate and On-Call rate to be discussed upon interview.
