RN Case Manager

Utilizes a multi-disciplinary plan of care to manage a defined population of patients/clients providing coordination of patient care which meets ACH's standards of quality, efficiency and desired outcomes. This position works under the general direction of the Director of Nursing. The position works one regular shift, 5 days per week but is responsible for resolving patient issues occurring on all shifts. This position is also responsible for being in Administrative weekend call rotation schedule. Incumbents may be subject to hours over 40 per week and callback.
