Your recruiter will provide the most up-to-date details during the hiring process._**How you'll make an impact in this role**+ Coordinateand serve as the primary leader of a multidisciplinary care team to ensure well-planned, holistic, and safe patient care.+ Implementand continuously monitor individualized patient care plans, driving improved health outcomes and adhering to all established clinical protocols.+ Provide compassionate, direct nursing care while serving as the patient's advocate to uphold the highest standards of quality and safety.+ Manage the patient's status, recording vital information and proactively communicating critical changes to the physician and healthcare team for timely intervention.+ Developand coordinate patient discharge plans, facilitating a successful and supported transition out of the facility and into post-acute care.+ Travel to other Urgent Care clinics in the Fox Valley area at least two weekends a month.**What minimum requirements you'll need**Licensure / Certification / Registration:+ BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.+ Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
