Job Opportunity: Licensed Practical Nurse (LPN)We are looking for a compassionate and committed Licensed Practical Nurse (LPN) to become a vital part of our Healthcare & Medical Services team. In this role, you will be instrumental in providing exceptional, resident-focused care within a collaborative, interdisciplinary setting. Your responsibilities will include delivering direct bedside nursing care to residents and patients, addressing their physical, emotional, and psychosocial needs while supporting our mission to transform healthcare through innovation, continuous improvement, and cultural enrichment.Key ResponsibilitiesConduct ongoing patient assessments and document vital signs accurately.Administer medications and treatments as prescribed.Assist with wound care, catheter management, and other clinical procedures within your state-approved scope of practice.Contribute to the creation and execution of personalized care plans.Monitor residents for any changes in their condition and report findings to the supervising RN or provider promptly.Participate in care conferences to ensure that each resident’s preferences and needs are met.Maintain accurate and timely documentation in the Electronic Health Record (EHR).Adhere strictly to infection control practices, safety protocols, and HIPAA regulations.Collaboration and CommunicationAs an LPN, you will work closely with RNs, CNAs, therapists, physicians, and other members of the care team to coordinate services, encourage early intervention, and promote positive clinical outcomes. You will also provide clear communication and education to residents and their families about medications, treatments, disease processes, and self-care strategies, empowering them to make informed decisions regarding their care.
