Hospice Certified Nursing AssistantSalary: $19-22/hr BOESchedule: Full-time | Monday- FridayShift Details: 8 AM-4:30 PMLocation: Bowling Green, Ohio, and surrounding areasRequirements of the CNA role:Licensed Nursing Assistant in the state of practice (CNA)CPR/BLS CertifiedPrior experience working in an AL/LTC/SNF facility or Home Health REQUIREDEnjoy working with Senior Citizens, Positive Attitude, Professional, Responsible & DependableMust be willing to travelValid driver's license and Auto insurance Coverage with reliable transportationHospice Experience PreferredAbout the Certified Nursing Assistant role:Provide compassionate, patient-centered personal care including bathing, grooming, dressing, toileting, and feeding in accordance with the hospice aide plan of care.Travel to patients’ residences, including private homes and facility settings, to deliver hospice services in a respectful and professional manner.Offer emotional support and companionship to patients and families, including spending quality companionship time to promote comfort, dignity, and quality of life.Provide bedside vigil support as needed during end-of-life care, ensuring patient comfort and presence during critical moments.Assist patients with ambulation, transfers, positioning, and range of motion exercises as documented in the hospice aide plan of care.Safely use and assist patients with durable medical equipment (DME) such as walkers, wheelchairs, hospital beds, and lifts.Observe and report changes in the patient’s physical condition, emotional status, or home environment to the case manager or supervising nurse.Accurately document care provided in accordance with Care 360 Hospice policies and procedures.Foster a cooperative team environment by understanding the roles of interdisciplinary team members and actively participating in team meetings and staff conferences.Provide support and education to family members and caregivers within the scope of the CNA role.Participate in required on-site monthly educational meetings and ongoing training to maintain competency and compliance.All other duties as assignedWhy work with Care 360 Hospice:Medical, dental, and vision health InsurancePaid Time Off401(k) MatchingLife InsuranceGrowth opportunitiesNEXT STEPSWe appreciate your interest in our Hospice Certified Nursing Assistant.Upon submission of your application, please be advised that we will reach out to you for further discussion should you meet the requirements of this position.Care 360 Hospice3130 Executive Parkway, Suite 500,Toledo, OH 43606Care 360 Hospice is an Equal Opportunity Employer M/F/D.
