Flexible PRN PT for Home & Community (Texas)

Physical Therapist (PT) – Contract (HCS Waiver) We are seeking a Contract Physical Therapist (PT) to provide skilled therapy services under the Texas Home and Community-based Services (HCS) Medicaid Waiver Program . This position offers a flexible, PRN-based schedule and is ideal for clinicians looking to supplement their current caseload.Compensation: $72.43 per hourLocation: Home and community settings (Texas)Schedule: Part-time / Contract (hours vary based on client needs)Position Overview The Physical Therapist will provide evaluation and treatment services to individuals with intellectual and developmental disabilities (IDD) enrolled in the HCS program. Services are delivered in home and community settings and focus on improving mobility, increasing independence, and enhancing overall physical functioning in accordance with HHSC and HCS waiver requirements .Key Responsibilities Conduct physical therapy evaluations and develop individualized treatment plansProvide skilled physical therapy services in home and community-based settingsImplement interventions to improve mobility, strength, coordination, and functional independenceCollaborate with interdisciplinary team (IDT) members , caregivers, and family membersMonitor client progress and modify treatment plans as neededMaintain accurate, timely documentation in compliance with HHSC, Medicaid, and HCS guidelinesParticipate in IDT meetings as requiredMinimum Qualifications Graduate of an accredited Physical Therapy program (Master's or Doctorate)Current, active Physical Therapist (PT) license in TexasAt least 1 year of experience in IDD, home health, pediatrics, or geriatrics (preferred)Strong knowledge of therapy practices and person-centered careExcellent communication and documentation skillsAbility to work independently in home and community settingsPreferred Qualifications Experience providing services within the Texas HCS or other Medicaid waiver programsFamiliarity with HHSC documentation and compliance standardsAdditional Requirements Current CPR certificationReliable transportation, valid driver's license, and auto insuranceAbility to manage a flexible and changing caseloadMust pass all required background and credentialing checksWork Environment Services are provided in client homes and community settingsFlexible, referral-based scheduling (PRN)#J-18808-Ljbffr
