Job OverviewWe are seeking an experienced Social Worker to provide direct services to pediatric and adolescent populations in a home care / school-based setting. This role involves working closely with students, families, and school staff to address social, emotional, and environmental challenges.Key ResponsibilitiesProvide direct social work services using the five phases of the social work processCollaborate with students, families, and school staff to develop effective care plansCoordinate care to ensure quality outcomes and efficient use of resourcesPerform case management and discharge planningRefer patients to community resources and agenciesSupport pediatric/adolescent populations with social and lifestyle challengesMaintain clear communication with all stakeholdersRequired QualificationsMaster's Degree in Social Work (MSW)Active LMSW or LLMSW License – Michigan5+ years Case Management experience2+ years recent Acute Inpatient Case Management (MANDATORY)Discharge Planning experienceValid Michigan Driver's License & insured vehicle
