Pharmacist Population Health
Position Summary: The Population Health Pharmacist partners with University Health Network primary care and network practices to improve medication use, close pharmacy related quality gaps, and advance value-based care outcomes. This part-time ambulatory role focuses on chronic disease management, cardiovascular risk reduction, medication adherence, medication access, and collaboration with providers and care teams across the network. Supports medication related quality improvement and population health initiatives * Identifies and addresses pharmacy related quality gaps within assigned primary care populations, including diabetes, hypertension, cardiovascular risk reduction, and medication adherence * Reviews patient level and panel level data to prioritize high risk or high opportunity patients for pharmacist intervention * Collaborates with providers and care teams to improve performance on quality measures and value-based care goals * Supports development and refinement of workflows that improve medication use, quality outcomes, and patient access across the network Provides comprehensive medication management and clinical support * Evaluates medication regimens for appropriateness, effectiveness, safety, adherence, and affordability * Makes evidence-based recommendations to optimize therapy and reduce medication related problems * Assists with medication initiation, titration, deprescribing, therapeutic interchange, and preventive pharmacotherapy consistent with protocols and scope of practice * Promotes cardiovascular risk reduction through optimization of therapies such as statins, antihypertensives, diabetes medications, and other guideline directed treatments Addresses medication access and adherence barriers * Identifies barriers related to cost, coverage, prior authorization, formulary restrictions, health literacy, and access * Collaborates with patients, providers, staff, and pharmacy resources to improve medication access and continuity * Supports adherence improvement strategies, including patient outreach, regimen simplification, and identification of access solutions Collaborates with providers and interdisciplinary teams * Works closely with primary care providers, nurses, care coordination, social work, and other team members to support integrated patient care * Communicates recommendations clearly and professionally within the electronic medical record and other approved communication channels * Contributes pharmacy expertise to care planning, transitions of care, and chronic disease management efforts across the network Documents clinical activities and interventions * Documents interventions, recommendations, and patient care activities in the medical record in accordance with organizational policies and professional standards * Ensures documentation is clear, concise, signed, dated, timed, and supports continuity of care and quality reporting needs * Tracks clinical interventions and outcomes as needed to support reporting, program evaluation, and value-based care initiatives Supports program development and quality performance * Assists with implementation of pharmacy services that align with primary care, population health, and value-based care priorities * Participates in quality improvement activities, committee work, and other initiatives that support University Health Network goals * Maintains current knowledge of evidence-based guidelines, payer quality programs, and best practices relevant to ambulatory and population health pharmacy services Position Qualification: Doctor of pharmacy from an ACPE Accredited College of Pharmacy and a Pharmacy Practice Residency from an ASHP accredited residency program; or equivalent combination of education and experience Licensed in the State of Tennessee is required
