Ambulatory Care Clinical Pharmacist - Family Medicine

The Ambulatory Care Clinical Pharmacist provides evidence‑based pharmaceutical care to patients in the ambulatory clinical setting. The delivery of care includes, but is not limited to, designing, implementing, assessing, monitoring, and documenting therapeutic plans using the best evidence; helping to achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams. When appropriate pharmacists in ambulatory care may perform patient assessments, participate in collaborative practice agreements, manage medication therapy, order and interpret medication‑related tests, coordinate care, provide education to patients and caregivers, and document care plans in the medical record. Zion Crossroads Ambulatory Care Clinic | UVA Health UVA Health is seeking an Ambulatory Care Clinical Pharmacist to join our Family Medicine team at an ambulatory care clinic in Zion Crossroads. In this embedded role, you’ll collaborate closely with physicians and care teams to deliver high‑quality, patient‑centered care. What You’ll Do Partner with Family Medicine physicians to co‑manage patients in an ambulatory setting Provide direct patient care, including comprehensive medication management Work within a Collaborative Practice Agreement to manage pharmacotherapy plans Optimize therapy for chronic disease states (e.g., diabetes, hypertension, hyperlipidemia) Educate and counsel patients to improve outcomes and adherence Identify and resolve medication‑related problems using evidence‑based practice Document clinical interventions in the electronic health record Why UVA Health Pharmacists are integrated clinical providers, not ancillary support Strong culture of interprofessional collaboration Opportunity to impact care at both the individual and population level Support for professional growth and clinical advancement Minimum Qualifications Education: Bachelor’s in Pharmacy with experience or Pharm.D. from an ACPE accredited College of Pharmacy. Degree required. Experience: PGY‑1 residency required or equivalent clinical experience. Licensure/Certifications: Commonwealth of Virginia State Board of Pharmacy License required. American Heart Association (AHA) Health Care Provider BLS certification required. Board Certification or equivalent certification specific to clinical area highly encouraged. Physical Demands Job requires sitting for prolonged periods; Repetitive motion: typing. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull. Onboarding Requirements Background checks and pre‑employment health screenings will be conducted on all new hires prior to employment. All safety‑sensitive positions with direct patient interaction also require a drug screen.
