Remote Chronic Care Health Coach

A company is looking for a Chronic Care Management-Health Coach. Key Responsibilities Develop patient coaching relationships to support care plans and quality of life Communicate abnormal findings to the cardiology care team and monitor patient adherence to care plans Serve as a liaison between patients and healthcare providers, identifying changes to reduce hospitalizations Required Qualifications High School diploma or GED 1 year of experience in remote chronic care management Experience with cardiology patients, specifically heart failure Registered Nurse (RN), Medical Assistant (MA), or Certified Nurse Assistant (CNA) certification Experience with EMRs, specifically Athena, NexGen, GEMMS, and e-clinical works
