We are seeking a BE/BC Geriatric Medicine Physician to join our PACE Interdisciplinary Team just north of Boston, providing comprehensive, goal-focused care to high-risk older adults in the community. Details: This is a full-time position, Monday- Friday 8am-4pm No nights, weekends or holidays! Participate as a member of the PACE Interdisciplinary Team Primary mission is to assist frail older adults to remain living in the community for as long as it is safe Bilingual is desired 1-year geriatrics experience and working with low income and diverse cultural groups preferred Salary Range: $200k to $294k Annually Responsibilities: Completes initial, periodic and annual physical examinations of the participants Evaluates participants for episodic visits, for acute illnesses and for follow-ups of hospitalizations in the clinic, at home or at a nursing home Makes nursing home visits in accordance with the state nursing home requirements and as needed Use telehealth as needed and where appropriate Participates in the PACE hospital rounding system and the on-call schedule Evaluates the needs of the participants and develops treatment plans in concert with the other members of the Interdisciplinary Team Perform other duties as required Must be comfortable with palliative and end of life care.
