DescriptionFull-time / Hybrid / Flexible Schedule Monday through FridayPublic Health Nurse Home Visitors are responsible for providing comprehensive community health-nursing services through home visits to high risk pregnant women, infants, children and youth in accordance with program goals, objectives and protocols.ESSENTIAL FUNCTIONS OF THE POSITION:The essential functions of this position require prioritizing and completing all assigned tasks in a timely and efficient manner, adjusting for changing priorities and availability of resources, and demonstrating initiative in identifying additional job-related tasks to be completed when time permits. These duties are a representative example of position expectations. Actual duties assigned may vary and change depending on the business needs of the department and the agency.Provides skilled public health nurse services at home or other community settings to medically high risk pregnant women, infants, children and youth with health risks or special needs.Uses professional nurse processes to develop family-centered plans of care to meet multiple, complex health needs and goals of population served.Provides health education and referrals appropriate to risk criteria identified for the client. Develops and maintains familiarity and knowledge of community resources and uses them appropriately to support client needs. Acts as a health advocate for/with families by collaborating with community agencies and promoting interagency coordination on behalf of and with the client.Maintains accurate, complete and confidential records that reflect assessment, services and outcomes while meeting established deadlines. Enters data in a timely way into the current data base or other data base required by public health established protocols.Participates in regular staff meetings, case conferences with Supervisor, reflective supervision, in-service training, formal training and other community meetings, as appropriate. Represent agency/program to the public; may provide presentations on nurse home visiting services.Respect the confidentiality of information about program participants, children and families, staff and personnel issues and agency operations. Report and document incidents of suspected child abuse and neglect to DHS and Supervisor within 24 hours of incident.Position requires ability to drive to and from community locations to provide these services and occasional travel for trainings and conferences.Provide consistent and sustained positive and measurable customer service experiences for internal and external customers resulting in increased customer/client satisfaction, effective dispute and conflict resolution, and positive public reviews.Provide exceptional and measurable customer service to internal and external customers with timely and accurate responses to their inquiries and concerns. Exercise tact, courtesy, and diplomacy when interacting with clients, contractors, vendors and others with whom we may work while enhancing the Company's public image at all times. Respond to all inquiries for information respectfully without regard to the inquirer's position, status, or demographic and in a manner that conveys understanding, acceptance and support for UCAN's programs and objectives.Communicate in a professional, respectful and courteous manner with all employees, clients, the Board of Directors, and others with whom we may work. Driving will be required for travel to training facilities, board meetings, events, client homes, community meetings, partner facilities, etc. and to perform other essential functions as needed.Complete designated job tasks, special projects and all other duties as assigned to meet team, department and agency goals while actively demonstrating accountability, high levels of employee engagement, and responsibility for achieving desired outcomes and measurable results.RequirementsKNOWLEDGE, SKILLS & ABILITIESQualified candidates for this position will have relevant education and experience necessary to perform the essential functions and meet the minimum performance expectations for this position with or without an accommodation.Experience working in public health, including home visitation, home health, antepartum, labor and delivery, postpartum, mother/baby, neonatal intensive care, and /or pediatrics, mental/behavioral health, or a minimumof 6 months of professional case management experience is desirable. Experience in providing maternal and child health services, particularly in a community/public health setting, is preferred.Demonstrated proficiency and experience with the theories and concepts of preventative health and skills in the application of nursing theory in Maternal Child Health and services to infants, children and youth with special health needs. Possess the ability to demonstrate respect for individuals and diversity by providing services using a non-judgmental, supportive approach; possess good listening skills; possess the ability to engage in mutual goal setting with families by using nursing process, assessment and interventions for problem solving and skill building in accordance with motivational interviewing and behavior change theory; possess the ability to learn and develop competencies in reflective practice; demonstrated sufficient competency in using Microsoft Office applications (Outlook, Word, Excel) in order to work successfully with department processes, procedures and practices.Demonstrated skill in interviewing and therapeutic relationship techniques, skill in use of computers and office equipment; use of medical assessment equipment. Skill in evaluating social service needs of families and linking families to appropriate resources in order to meet identified needs. Skill in producing appropriate and adequate nursing documentation.Demonstrated ability to communicate effectively in both oral and written forms; make decisions independently in accordance with established policies, and use initiative and judgment in carrying out tasks and responsibilities; establish and maintain records, patient charts, reports, and statistical data; represent program in a variety of settings; maintain confidentiality; remain calm and use good judgment during confrontational or high pressure situations; courteously meet and deal effectively with coworkers, children and families, federal and state agencies, consultants, medical providers, community groups and the public. Ability to use electronic timekeeping system is required.Required Licenses & CertificationsValid driver's license with a driving record that meets UCAN's acceptable driving guidelines.Must pass criminal background check prior to hire and successfully meet all screening standards when required by department or program funding source(s).Minimum Education & ExperienceAssociates degree in nursing.Current CPR BLS Certification.Current unencumbered Oregon Registered Nurse License.Preferred Education & ExperienceBachelor degree in nursing.Coursework emphasis in public health and experience in a public health setting desired.Consideration may be given to an Associates or Bachelor degree in nursing with related areas of education and experience with an encumbered Oregon Registered Nurse License.An equivalent combination of experience and education that demonstrates the required knowledge, skills and abilities required for the position will be considered in lieu of the outlined requirements.PHYSICAL DEMANDSThe physical demands described below are representative of those that must be met by an employee to successfully perform the essential functions of this position.
