Assists with ensuring proper medical equipment and supplies are ordered and communities are prepared for clinical needs of patients transitioning from acute care as well as entering referrals, leads, insurance verification and other required information into CRM.Education/Qualifications:Must have a working clinical knowledge of medical terminology and have exceptional knowledge of SNF clinical capabilities.Must have the ability to read, interpret and analyze referral information to provide appropriate response and have sufficient knowledge of financial payers and pre-cert requirements.Must possess basic computer skills and ability to navigate through various software programs, including referral software and electronic health records software.Shifts and Wages:Full-Time Opportunity AvailableShift Needed: DaysWage: $60,000 - $70,000 per year*Program details can vary amongst buildings.
