We empower our employees with the resources, incentives, and flexibility that they need to support a healthy, balanced, and fulfilling career by providing many valuable benefits and a balanced compensation structure combined with career development.Position DetailsTitle: REMOTE RN Case Manager Location: Michigan (100% Remote) Position Type: Contract – 12 Months (Possible Extension)License Requirements : MI State RN License Work Model: Fully Remote (No onsite requirement)Position SummaryLead coordination of a multidisciplinary team to deliver holistic, person-centric care managementAct as the single point of contact for members, caregivers, and providersCommunicate via phone, email, text, and online messaging platformsApply the case management process to assess, plan, implement, monitor, and evaluate care plansCollaborate with members, providers, and community resources to improve health outcomesKey ResponsibilitiesCoordinate a regionally aligned multidisciplinary team (Social Workers, Dietitians, Pharmacists, Clinical Staff, Medical Directors)Assess, develop, implement, and evaluate individualized care plans across the care continuumEvaluate members’ clinical, psychosocial, cultural, and support needsEngage members/caregivers to identify care gaps, address barriers, and improve outcomesArrange and coordinate resources such as community services, mental health, substance abuse, and financial support programsFacilitate collaboration between providers, family members, and community agenciesAdvocate for members and promote self-advocacyProvide education on health literacy, self-management, medications, and nutritionMonitor care plan effectiveness and adjust based on progress and adherenceMaintain accurate and timely documentation of all member interactionsSupport care transitions including discharge planning and coordination of servicesEducate members/caregivers on post-care instructions and follow-up requirementsArrange durable medical equipment (DME) and transportation servicesEnsure communication across all care stakeholders during transitionsAdhere to professional standards, protocols, and productivity goalsMaintain ongoing professional development and certification requirementsEducation & ExperienceNursing Diploma or Associate Degree in Nursing (required)Bachelor’s Degree in Nursing (strongly preferred)Minimum 3 years of clinical nursing experience (acute, post-acute, or community settings)1+ year of case management experience in managed care (preferred)Experience with telephonic and digital patient management (preferred)Certifications & LicensureActive, unrestricted Michigan RN license (required)Certified Case Manager (CCM) certification (required within 18 months)Chronic Care Professional (CCP) certification (preferred)Skills & CompetenciesStrong critical thinking and problem-solving abilitiesEmpathetic approach with strong listening skillsProficiency in motivational interviewingEffective time management and multitasking abilitiesStrong organizational skills with ability to handle multiple systems/toolsIntermediate computer skills (Excel, Outlook, Teams, Word, OneNote)Ability to work independently and within a team environmentExcellent written and verbal communication skillsLocation: Michigan REMOTE Disclaimer: Kyyba is an Equal Opportunity Employer.
