This role is essential in ensuring high-quality, compassionate care while supporting the overall health, safety, and well-being of residents.Key Responsibilities:Deliver direct patient care, including medication administration, treatments, and ongoing monitoringEducate patients on medications and care plansMaintain accurate and timely documentation of all care providedPerform computer-based data entry (training provided)Ensure full compliance with all regulatory and accreditation standards, including:ICF-IID RegulationsDSSLC Policies and ProceduresHHSC Standards and GuidelinesCollaborate with interdisciplinary teams to support patient outcomesQualificationsActive LVN licenseAt least 2 years of recent LVN experience requiredMust be able to pass a background check and Drug Test
