Based in Crystal Lake, IL, the Physical Therapist will assess, plan, and execute individualized treatment programs for patients receiving in-home care. Primary responsibilities include evaluating patient conditions, implementing rehabilitation plans, focusing on improving mobility and functionality, and providing guidance to patients and their families. The role involves collaboration with a multidisciplinary team to ensure patient-centered care and successful recovery outcomes. This job will require travel to patients homes in Crystal Lake and neighboring cities. QualificationsProficiency in Physical Therapy practices, Rehabilitation techniques, and Home Care servicesStrong knowledge of Medicine and Patient Care standards in a home health settingExceptional interpersonal and communication skills to interact effectively with patients, families, and healthcare teamsValid Physical Therapy license in the state of IllinoisAbility to manage time effectively while handling multiple tasks in a varied work environmentCPR certification Experience with electronic medical records (EMR) systems is beneficial
