Home Health Physical Therapist

Job Overview Forest Park Home Health is seeking a dedicated and compassionate Home Health Physical Therapist to join our healthcare team. This role involves providing personalized physical therapy services to patients in their homes and community settings, promoting recovery, mobility, and independence. The ideal candidate will have a strong background in physiotherapy, with experience across various patient populations including geriatrics, orthopedics, and acute care. Responsibilities Develop and implement individualized care plans tailored to each patient’s needs, including discharge planning and goal setting. Conduct comprehensive assessments of patients’ physical condition, including physiology and anatomy evaluations. Deliver physiotherapy interventions such as trigger point therapy, orthopedics, and outpatient treatments to improve mobility and reduce pain. Manage patient care across diverse settings including inpatient hospitals, outpatient clinics, and home health environments. Coordinate with healthcare teams to ensure seamless continuity of care and effective discharge planning. Document patient progress accurately using EMR systems and maintain detailed records of treatment plans and outcomes. Educate patients and their families on exercises, safety measures, and health maintenance strategies to promote long-term wellness. Monitor patient responses to therapy and adjust treatment plans accordingly to optimize recovery outcomes. Stay current with medical terminology updates, physiotherapy techniques, and best practices within home & community care. Qualifications Valid state licensure as a Physical Therapist with current registration. Proven experience in home health settings, inpatient hospital environments, outpatient clinics, or similar healthcare facilities. Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques including trigger point therapy. Experience with discharge planning, care plans development, and working with diverse patient populations such as pediatrics and geriatrics. Familiarity with EMR systems for documentation and patient management. Excellent patient care skills with the ability to assess needs accurately and deliver compassionate treatment. Knowledge of orthopedic rehabilitation, outpatient therapy protocols, hospital procedures, and community-based care services is preferred. Ability to work independently while collaborating effectively with multidisciplinary teams.
