Travel- Social Worker - CRC - 20904

This position requires providing service to those presenting to the Crisis Response Center through the following activities: complete bio-psycho-social assessments from a strengths based perspective; recommends services in the community which will support those being discharge from the CRC; identifies and implements interventions for frequent utilizers of the CRC and Emergency Department; conducts follow up calls to ensure compliance with aftercare.
