Registered Nurse
Job Summary: The Registered Nurse will provide skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Responsibilities include patient assessments, medication administration, treatment planning, and collaboration with an interdisciplinary team. The role requires strong communication and collaboration with patients, providers, and team members, with a minimum of 2 years of experience.Location:Santa Rosa, California, United StatesResponsibilities:- Provide skilled nursing care.- Perform patient assessments.- Administer medications.- Develop and implement treatment plans.- Collaborate with the interdisciplinary team.- Communicate effectively with patients, providers, and team members.- Cover night shifts as needed.- Float between similar units as required.- Participate in on-call duties, distributed equally among nursing staff every 6 weeks (potentially 6-8 shifts in a 6-week period, including nights, evenings, and weekends/8-hour shifts).Required Skills & Certifications:- Clinical Assessment, Medication Administration, and Treatment Planning skills- Minimum of 2 years of experience in hospital, home health, or hospice care- Experience in trauma, neuro, vascular, and ortho- Interdisciplinary Team Collaboration skills- Excellent communication and interpersonal skills- Ability to work effectively in a fast-paced environment- Active Registered Nurse license in the state of working- Bachelor's degree in Nursing or related field- BLS certificationPreferred Skills & Certifications:- Not specified.Special Considerations:- May need to cover one night shift within the contract.- Weekend work is required.- On-call is required and will be distributed equally amongst all nursing staff every 6 weeks.- Trauma center environment.Scheduling:- Variable start times: 7:00 AM, 9:00 AM
