Through a membership based approach, patients are supported by an integrated care team—including a physician, dietitian, physical therapist, and care navigator working together to help patients fully bloom. We combine clinical care with personalized exercise plans and nutrition coaching, using technology and continuous support to help people make lasting changes to how they live, move, and eat.The OpportunityWe are seeking a passionate Registered Dietitian (RD or RDN) to join Bloom360 as an independent contractor within a fully virtual, preventive primary care model. You will work with a dedicated panel of patients to deliver personalized, longitudinal nutrition care that is integrated with medical care and focused on improving clinical outcomes.Your work will support prevention and management of cardiometabolic disease, including obesity, diabetes, hypertension, dyslipidemia, and gastrointestinal conditions, while also promoting overall vitality and longevity. This role emphasizes evidence-based nutrition therapy, behavior change science, and close collaboration with physicians to ensure nutrition is fully embedded into each patient’s care plan.What You'll DoConduct comprehensive virtual nutrition assessments, including dietary intake, metabolic risk factors, social determinants of health, and readiness for changeDevelop and implement individualized, evidence-based nutrition care plans aligned with medical diagnoses, lab values, and physician treatment plansDeliver ongoing telehealth counseling using behavior change frameworks (e.g., motivational interviewing, stages of change) to support sustainable lifestyle modificationCollaborate closely with Bloom360 physicians and care team to co-manage patients with chronic conditions (e.g., diabetes, hypertension, obesity, metabolic syndrome, GI disorders), ensuring nutrition interventions are coordinated with medical careMonitor patient progress using clinical outcomes (e.g., A1c, lipids, weight trends, blood pressure) and adjust care plans accordinglyProvide nutrition support across a range of needs, including weight management, cardiometabolic health, disordered eating recovery, prenatal/postpartum nutrition, and gastrointestinal healthMaintain accurate, timely, and clinically meaningful documentation in the EHR, aligned with interdisciplinary careParticipate in care team case reviews, care coordination, and quality improvement initiatives aimed at improving population health outcomesWhat We're Looking ForRequired Qualifications:Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential — active and in good standingLicensed in the state of Michigan (or eligible for licensure)Minimum 2 years of clinical or counseling experience post-registrationStrong comfort with telehealth and virtual care deliveryDemonstrated experience managing cardiometabolic conditions (e.g., diabetes, cardiovascular disease, obesity, metabolic syndrome) using evidence-based nutrition interventionsStrong skills in motivational interviewing, behavior change counseling, and patient-centered communicationComfort interpreting and applying clinical data (labs, medications, diagnoses) to nutrition care plansPreferred Qualifications:Experience in a Direct Primary Care, concierge medicine, or membership-based health modelCertified Diabetes Care and Education Specialist (CDCES) or similar specialty certificationBackground in intuitive eating, anti-diet approaches, or Health at Every Size (HAES) frameworksCultural competency experience — comfort serving diverse patient populationsBilingual (Arabic, Spanish, or other languages) is a strong plusFamiliarity with HIPAA-compliant telehealth platforms and EHR systemsWhat Makes You a Great Bloom360 FitYou genuinely believe food is medicine.
