Licensed Clinical Social Worker (Health Coach) – Richmond, VAThank you for considering a career at PartnerMD!PartnerMD, a membership concierge medical practice specializing in primary care and executive health, has an opportunity for an LCSW-trained Health Coach in Richmond, VA. This LCSW Health Coach is responsible for facilitating individual and group programs that are part of the Wellness University curriculum and that include behavioral health education and brief therapeutic counseling on specific topics.ESSENTIAL ROLE OBJECTIVES: Facilitate Wellness University courses that provide education and coaching to PartnerMD patients and corporate executives:Facilitate programs in the schools of Cognitive, Adrenal, Muscular, and Metabolic health that have some behavioral health or therapeutic elementProvide focused education and therapeutic support appropriate to the Course topic (e.g. Parkinson’s caregiver support, insomnia, stress management, binge eating, smoking cessation) Primary care environment (e.g. 3-4 sessions and referring to outside specialists when appropriate)Work in conjunction with the mission and vision set out by the Director of WellnessConduct sessions both in-person and via telemedicine platformCreate personal and course-specific behavioral wellness plansAssist in the creation and implementation of new Wellness University programs related to behavioral health (courses, support groups, etc.)Motivate members to adopt a healthy lifestylePromote wellness programs for current and new membersSchedule follow up appointmentsDocument sessions in our electronic medical records systemCommunicate with physicians and other staff regarding sessions and follow up care Assist with patient/member engagement activitiesEngage members via social media postings that promote wellness programming (Facebook Live, private Facebook Groups)Lead webinars and support groups on wellness topics Maintain and administers schedule in EMR (blocks, freezes, etc.) to effectively manage tasks and retain patient-centric flexibility Collaborate with a team of health coaches from multiple states, the Wellness Program Coordinator, and the Director of Wellness to develop and implement new wellness programmingQUALIFICATIONS: Master’s degree in social work or related field preferred At least two years of prior experience as Licensed Clinical Social Worker Active LCSW license in Virginia, with ability to be licensed in additional states Ability to be credentialed with major insurance carriers and Medicare Clean malpractice record Ability to multitask Highly organized and detail-oriented Strong interpersonal skills with an ability to relate to people at all levels of an organization and with differing levels of health awareness with professionalism and integrity A team player Strong computer experience and fluency with electronic medical record (EMR) Experience using social media for professional work (preferred)REPORTING AND OTHER EXPECTATIONS:The position will report to the Wellness Director and will have significant contact and involvement with other members of the management and administrative team and with the general public. The work will be based at PartnerMD’s office in Richmond, VA.A non-smoking candidate is preferred.
