(APP) Nurse Practitioner / Physician Assistant - Alamo Heights
POSITION OVERVIEWHarbor Health is seeking a team-oriented Nurse Practitioner or Physician Assistant to join our multi-specialty clinic group in San Antonio at our Alamo Heights practice! As a clinical leader within our primary care setting, you will provide effective, efficient, and appropriate services to our members while demonstrating safe clinical decision-making and expert care. You will work collaboratively with our clinical practice team to deliver exceptional care that aligns with our mission of transforming healthcare through our payvider model.POSITION DUTIES & RESPONSIBILITIESInterview members to obtain medical history, perform physical examinations, order laboratory and other diagnostic tests, prescribe medications, and recommend treatmentsProvide continuity in managed care for members with pre-existing long-term conditionsMake referrals for secondary and tertiary care as needed in collaboration with the care teamPerform medical procedures according to issued privilegesProvide health maintenance visits, evaluate immunization needs, offer anticipatory guidance and referralsPerform contract and special physical examinations for employment and other purposesConduct STD screening, family planning, and health educationDocument and code all member contacts accurately and legibly in the electronic medical recordFunction as an active member of the medical teamParticipate in and manage care team meetingsContribute to organizational initiatives, including participation on clinical committees, providing education, quality review, and other mission-aligned activitiesDESIRED PROFESSIONAL SKILLS & EXPERIENCERequired:Graduate of an accredited Physician Assistant Program and certified by the National Commission on Certification of Physician Assistants (NCCPA) or Nurse Practitioner Program and certified by the American Nurses Credentialing Center or the American Academy of Nurse PractitionersCurrent licensure as a PA/NP to practice in the State of TexasLicensed and certified to practice in Family MedicineCurrent DEA and CPR certificationValid Texas Driver's LicenseAbility to meet and maintain current organizational credentialing and privileging requirementsComfortable working as a solo provider in a clinic team settingComputer literacy with ability to navigate electronic health record systemsExcellent customer service skills and professional communicationAbility to interact effectively with persons from diverse backgroundsStrong teamwork capabilities while maintaining ability to function independentlySound judgment, flexibility to shift priorities, and commitment to confidentialityPreferred:2+ years' experience as an Nurse Practitioner or Physician Assistant in a primary care settingBilingual in English/SpanishPhysical Requirements:Ability to work irregular hours in a moderate-stress environmentCapacity to handle exposure to blood, body fluids, communicable diseasesAbility to push/pull heavy objects and lift up to 50 poundsFull range of body motion for patient care activitiesWHAT WE OFFERCompetitive salary and incentivesGenerous PTO10 paid holidaysCME Allowance and time offMedical, Dental and Vision InsuranceOccurrence-based malpractice401k Investment PlanCompany EquityAt Harbor Health, we're transforming healthcare in Texas through collaboration and innovation.
