Organization: Path Home Job Title: Outreach and Housing Navigator (Pilot Program) Reports To: Housing Program Manager Job Specifics: Full time at 40 hours per week, salary Compensation: $54,000 annually or $56,000 annually if bilingual in English and Spanish Benefits: Full health and vision insurance (Providence) and dental insurance (Moda). Option to add dependents at employee's pre-tax expense. Path Home will contribute an amount equal to 2% of employee's total annual salary into a Simple IRA. Eligible for Employee Assistance Fund and paid Sabbatical after 7 years. Paid Time Off: Three weeks of Paid Time Off (120 hours) plus one week of Paid Sick Time (40 hours) annually. Overview: The Outreach and Housing Navigator position is a new role at Path Home and is part of a multi-agency team that provides street and site-based outreach to homeless families with children with a goal of helping families move from homelessness into permanent housing. The position is hybrid and mobile in nature and works amongst a team of community service providers who serve families experiencing homelessness. Work focuses on building relationships, trust, and support in order to promote the strengths and capabilities of families experiencing homelessness. You will be expected to work in a “mobile office” that requires familiarity with the geography of Portland and you’ll also have a home base at our Family Village Campus on SE 92 & Woodstock. The Outreach and Housing Navigator will provide information and referral navigation to other services as appropriate and ensure all families have completed the required screening and engagement to access Multnomah County Homeless Family System of Care. The project is delivered through a partnership between Path Home and El Programa Hispano Católico (EPHC), Our Just Future, Self Enhancement Inc (SEI), and Native American Youth & Family Center (NAYA). The Outreach and Housing Navigator will also receive an allocation of client assistance funds to support participants with obtaining documents collection, barrier mitigation, housing-related application fees, and diversion. Specific Job Duties: Assess families for vulnerability and assist them in accessing community resources and housing programs. Build relationships with parents and children experiencing homelessness, ensuring services are welcoming to all diverse families. Provide immediate support for families on the streets, helping them access shelters and rapid re-housing resources to transition back into permanent housing. Ensure program eligibility requirements are met and aim to place at least 20 households into permanent housing within our current fiscal year (ending June 30, 2025). Manage a budget to support families in moving from homelessness to housing. Keep detailed financial records and receipts that will be used for government contract compliance and billing. Develop case plans for families experiencing trauma and homelessness to support them in achieving long-term housing stability. Ensuring accurate and timely data entry for all client-related data and case notes into Apricot and HMIS, and other cloud based data collection data quality standards are met. Collaborate effectively with a diverse team, participating in team-building, meetings, and organizational functions. Skills and Abilities: Respond compassionately and non-judgmentally to families in need, maintaining composure in stressful environments. Uphold strict confidentiality and professional boundaries with households. Communicate clearly and concisely, both verbally and in writing. Build respectful relationships with diverse families, directly interacting with children daily. Support families in navigating Portland and Multnomah County resources. Understand the impacts of trauma on homelessness and poverty. Use computers and mobile devices proficiently for communication and documentation. Learn new skills and adapt to different situations flexibly. Work independently and collaboratively with the Path Home team and system partners. Minimum Qualifications: Experience working with individuals or families experiencing homelessness and a knowledge of local social services is a plus. Must be reliable and show up to work on time during every scheduled shift. Must be self-motivated and work well in an unstructured environment. Experience with crisis intervention and customer service. Personal vehicle, valid driver’s license, and insurable driving record required upon hire (Path Home will pay for mileage). Familiarity with computers and software such as Google Suite (G Suite) as well as the ability to utilize computers and mobile devices to access information via the internet is necessary.
