Faith Based Case Manager - Part Time (20 Hours Per Week)
Summary:
The goal of the Case Manager is to assess housing, financial, mental health, spiritual, and physical health needs of residents in the Life Transformation Program and make appointments and find resources to help them.

Essential Duties and Responsibilities include the following and other duties may be assigned:
· Help the discipleship of clients in the growth of their faith and their walk with the Lord.
· Assess the mental and social needs of the clients. Make Initial Health Assessment Appointments for residents.
· Crisis intervention as needed.
· Experience working with persons in crisis.
· Good documentation skills.
· Help clients locate and access other resources.
· Mediate conflicts.
· Participate in staff meetings to update client’s status.
· Participate in team decisions about placement of clients in the Life transformation Program.
· Process good interceptional skills.
· Schedule weekly appointments with clients
· Supervise and provide direct accountability for all Program clients.
· Assist in needed assessments.
· Prepare Life Management Fact Team referrals for clients who may qualify for the FACT Team and forward to the appropriate Life Management Team Case Manager for processing.
· Provide Clothing and product as needed to residents.
