Growing together.To provide a variety of patient care activities supporting the needs of the Care Coach Connect Program in clinical duties in patients' homes as well as some office/clerical duties.Position in this function organizes the clinical environment and provides support in patient care situations. Support includes, but is not limited to, assisting physicians and nursing personnel, including those skills listed under Job Functions below along with various other procedures under the direct supervision and responsibility of a Advance Practical Clinician or medical provider. The CMA assists in identifying patient needs or problems and communicating data to the provider or other members of the clinical team. The CMA delivers quality customer service. Ensures policy and procedures are followed including infection control, privacy and confidentiality.Primary ResponsibilitiesPerforms all duties within the scope of a Certified Medical Assistant (Items included are provider supervised procedures, injections, diagnostic testing, phlebotomy)Coordinates healthcare screening results as directed by providers, company standards, and policiesTravels with provider to patient homes, mobile units, and other locations within the NTX Market and surrounding NTX Market to support the delivery of (departmental) services in the community settingPerforms and completes all quality program health screenings per required metricsProcesses appropriate documents in an organized and accurate manner and copies all physicians on patient results and quality forms submittedRecords patient care documentation in the medical records accurately and in a timely matter according to company policyReviews all medical information, test results and suvey questions prior to the patient leaving to ensure all information is capturedEnsuring that supplies are available, stock is inventoried, log data is accurate and supplies are rotated. Follows up with discrepanciesPresents abnormal results to the provider promptly to ensure there is not an emergent condition that needs immediate attentionPost visit preparation of the clinical team charts for faxing, filing and forwarding to the PCP officesResponsible for ensuring the completion of compiling results and entering chart noted prior to visit. This includes, but is not limited to, pull hospital records, lab results, specialty notes, and etc.Ensures that follow up appointment dispositions are documented in CDSS and completed In eCWReviewing and ensuring that post visit recommendations from provider are completed such as prescriptions and referralsMaintains patient confidentiality of all materials handled within the department as well as proper release of patient information at all timesMaintains certifications (MA and CPR) and quality control standards Promotes positive work environmentMay assist other members of their team with coordination of work Performs all other related duties as assignedOperates diagnostic equipment (cannot interpret tests), assists in removal of staples from superficial wounds, changes wound dressing and obtains cultures, administers non intravenous medication performs simple specimen collection via noninvasive techniques and collects blood specimens via venipuncture or via capillary, performs EKGs.
