Family Support Specialist
Job Summary
The Family Support Specialist manages a case load and provides case management services and career coaching to individuals residing at HEARTH. The Family Support Specialist utilizes trauma-informed care, motivational interviewing and harm reduction best practices in order to provide the highest level of care to all families.
Duties:
● Provide case management services to individuals residing at HEARTH on a weekly basis, at a minimum
● Support families residing in Benet Woods and HEARTH's permanent housing locations
● Create service plan and budgets monthly with each family to include housing, income, employment, education, health and family goals
● Manage client case file, documentation, and case notes
● Oversee client progress toward self-sustainability
● Coordinate trainings and services to adults and children
● Represent HEARTH at external events
● Input and maintain participant information and progress in database
● Coordinate supports that will assist the participant with attendance and participation in program activities and make referrals to partnering agencies, as appropriate
● Ensures confidential maintenance of all files
● Support on-site Monday night programs, as needed
● Remain up to date on services applicable to families at HEARTH including mental health services, transportation programs, employment, and education resources
● Develop and maintain a knowledge of topics related to parent requests through use of literature, personal contacts, electronic resources, and professional development
● Encourage, model, and coach appropriate early childhood practices, parenting skills and behavior management/modification techniques.
● Locate housing, establish and maintain relationships with landlords or property managers, and facilitate execution of leases
● Advocate on behalf of participants to promote housing quality and stability
● Complete drug and alcohol testing, as necessary
● Provide transportation assistance through the HEARTH vehicle and resources for transportation services
● Participate in the on-call rotation
● Performs other duties as assigned.
Qualifications:
● Bachelors Degree in Social Services, Counseling, Social Work, or related field, preferred
● Effective interpersonal skills and presence, including articulate oral and written language skills
● Valid driver’s license
● Must be able to successfully pass a pre-employment drug/alcohol screen and background investigation. Any satisfactory equivalent combination of experience and training which ensures the ability to perform the work may substitute for the above so that the employee will possess the abilities or aptitudes to perform each duty proficiently
Preferred education and experience
● Social Work and Counseling experience preferred.
● Licensed in Social Work or Counseling preferred.
● Facilitation of connecting clients with other services in the community experience.
