Case Manager
Position Summary: Identifies client’s needs by assessing client situation and availability of resources
Educational Requirements: High school diploma or equivalent (required); B.A. in Social Work or related field (preferred)
Special Skills: Basic knowledge of substance abuse treatment; counseling experience; working knowledge of community resources; advocating skills; strong interpersonal communication skills and professional boundaries to provide excellent internal and external customer service.
Position Duties:
Coordinates treatment and social services available to client by interviewing client, exploring needs and goals of client, assisting client in completing applications and in the overall navigation of internal and external systems of care that meet the persons basic human needs.
Assists clients in the development of an individualized care plan to determine sustainable SMART goals.
Acts as a liaison between clients and providers through resource education, program overview, client rights, treatment interpretations, and determinations.
Connects clients to community resources such as housing, medical, educational, and vocational services.
Provides services that focus on improving social functioning through an integrated recovery approach that improves daily living skills and supports clients in meeting their goals.
Provides emergency/crisis intervention services by referring client to other appropriate programs.
Complies with federal, state and local legal requirements by studying existing and new legislation; informing management of needed actions.
Maintains customer confidence and protects operations by keeping client information confidential; following established confidentiality policy and procedures; maintaining professional boundaries with clients.
Maintains inter- and intradepartmental work flow by fostering a spirit of cooperation.
Displays a high level of professional ethics, boundaries, and clinical expertise with complex needs populations.
Completes periodic outcome measures with clients.
