This contract position focuses on delivering high-quality rehabilitation services to patients who need to improve mobility, regain strength, and achieve greater functional independence.Schedule (Preferred): Sunday through Thursday or Tuesday through Saturday May require floating to a nearby sister facility (within walking distance) Qualifications: Current and active Physical Therapist license in California Skilled nursing facility experience preferred; mentorship available for those with 0-1 years of experience Demonstrated proficiency in documentation and time management Adaptable and collaborative with a patient-centered approach Key Responsibilities: Conduct comprehensive evaluations of patients admitted to the skilled nursing facility Design, implement, and adjust individualized treatment programs to support mobility and daily functioning Collaborate closely with the interdisciplinary care team to deliver coordinated care and maximize patient outcomes Complete timely and accurate documentation using Net Health software within facility guidelines Maintain open communication with patients, families, and colleagues, ensuring the highest quality of rehabilitative care This opportunity provides a supportive environment with professional mentorship for newer therapists and the professional growth found in a well-established, multidisciplinary care team.
