Care Manager, Remote (LPN)

Salvo Health is looking for an experiencedLPN/LVNto support our chronic disease patients. In this role, you'll be a key contributor to the management and delivery of our care program, working closely with our patients, registered dietitians, and partner physicians to provide best‐in‐class care.You'll use your medical knowledge and product expertise to help our members navigate Salvo Health, assisting in medical and administrative tasks to keep the member moving through the program. Additionally, you'll help shape how we build and scale our product and process.Our ideal candidate is someone who enjoys the challenges of an early‐stage start up, is eager to learn, process oriented, and has a patient‐first mindset.What You Will DoProvide exceptional care, disease management and health education to patientsSupport goal setting for individual patients asynchronously to help them better manage their chronic conditionsCreate personalized action plans with guidelines to reduce or eliminate unwanted behaviorsHelp clients understand their motivations and create behavior change plansConduct regular outreach to patients, based on their needs and preferences, to support engagement and elicit behavior changeCoordinate with other clinical team members to provide an exceptional patient experienceDevelop and maintain professional, support‐oriented working relationships with patients and team membersCreate and distribute health education materials to individual members as necessaryWork with a cross‐functional product team to develop and constantly improve our in‐app patient experienceQualifications2+ year of experience as Licensed Practical Nurse or any Nursing license2+ years of experience in patient‐facing or customer‐facing rolesCompact state license required, additional licensing may be neededBilingual (Spanish speaking) a plusExcellent customer relation skills, as well as written and verbal communication skillsKnowledge of medical terminology and proficiency of general medical office proceduresFamiliarity with digital applications like Slack, Coda, Google Workspace, etc.Strong analytical and proactive problem solving skillsSelf‐motivated, results‐oriented and strategic thinkerPersonal passion for health and wellness topicsMust be authorized to work in the United StatesExperience working in telehealth or healthcare startup environment preferredBonus: Experience working in GI, weight management, and/or with anti‐obesity medications such as GLP‐1sCompensation $25 - $26 an hourBenefits Salvo offers a competitive salary and health benefits, a remote work environment, flexible time‐off, and professional development and entrepreneurial opportunities.Equal Opportunity Employer Statement Salvo is committed to creating a diverse environment and is proud to be an equal opportunity employer.
