Licensed Social Worker Location: Lodi, CA Agency: Cynet Health Pay: $2,295 per week Shift Information: Rotating - 4 days x 10 hours Contract Duration: 13 Weeks Start Date: 4/6/2026 About the Position Job Title: Social Worker Profession: Social Worker Specialty: Social Worker Duration: 13 weeks Shift: Day Shift Hours per Shift: 10 hours Experience: One year experience in an acute or long-term medical setting required; two years preferred License: Master’s Degree in Social Work required Certifications: None specified Must-Have: Experience in providing support and assistance to patients and families Description: Provides services to patients and their families to address needs related to hospitalization. Offers counseling, support, and assistance in developing transition plans. Guides the routine aspects of non-medical issues for patients and their families. Assists with planning and post-discharge arrangements with outside agencies, nursing staff, and relatives. Collaborates and facilitates patient discharge plans for those requiring post-Hospice or post-inpatient care. Helps patients, families, and caregivers determine the appropriate level of Hospice care. Refers to outpatient resources as necessary. Initiates a plan of care and conducts appropriate patient teaching. Reassesses the plan of care based on the physical and emotional needs of the patient. Develops a Plan of Care (POC) based on patient assessment, including psychosocial factors that may impede the POC, knowledge of human development stages, the impact of terminal illness, mental status, and family dynamics.
