This role is an integral, in-office member of our clinical care team, working alongside fellow Care Coordinators and collaborating with Ennoble Care providers within a family-practice-style model of care.While not patient-facing, the position plays a critical role in supporting patient-centered care coordination, documentation, and communication, helping ensure continuity of care and timely follow-up across care settings.At Ennoble Care, we foster a positive, respectful culture rooted in teamwork, where individuals are valued and encouraged to do meaningful work while maintaining a healthy work-life balance. We support internal advancement and long-term career growth based on performance, organizational needs, and role availability.Key ResponsibilitiesCoordinate patient-centered care activities using Ennoble Care’s electronic medical record (EMR) systemSupport Ennoble Care providers and clinical teams through accurate documentation, follow-up tracking, and care coordinationServe as a key point of communication between providers and internal care teams regarding clinical updates, patient needs, and care coordination prioritiesCommunicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely and appropriate mannerAssist with care transitions, referrals, and coordination with external partners, as neededCollaborate closely with fellow Patient Care Coordinators and internal care team members within an office-based environmentMaintain accurate, timely documentation in accordance with established workflows, documentation standards, and escalation protocols to support timely decision-making and efficient clinical operationsContribute positively to team collaboration, workflow efficiency, and day-to-day office operationsRequirementsLegally authorized to work in the United States of AmericaActive, unrestricted clinical certification or licensure, such as:Patient Care Technician (PCT)Certified Nursing Assistant (CNA)Medical Assistant (MA) / Certified Medical Assistant (CMA)Licensed Practical Nurse (LPN)Registered Nurse (RN)Or comparable credentialMinimum 1 year of experience in a clinical and administrative or care coordination role within a healthcare settingComfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and internal systemsCultivate and maintain positive, approachable working relationships while demonstrating a consistent, dependable presenceWell-developed organizational, time-management, and attention-to-detail skills, paired with an optimistic approach and initiative-driven mindsetStrong working knowledge of healthcare terminology and care coordination principlesProficiency with Microsoft 365, including Outlook, Word, and ExcelAbility to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, in an office-based setting in the Voorhees, New Jersey areaNo weekendsNo on-callDaytime hoursSeven (7) company-paid holidays annuallyAbility to pass a background check and drug screenFull-time benefits eligibility with a variety of options, including a Wellness Pet Plan for your furry family members!Please answer all application questions thoroughly.
