This is a high‑impact role for a seasoned professional who excels in relationship development, referral growth, and strategic outreach within the healthcare community.If you are a motivated liaison with proven Medicare referral success and established relationships across hospitals, SNFs, and physician groups, we want to speak with you.About the RoleAs a key connector between our home health program and the broader healthcare network, you will:Build and strengthen referral relationshipsDrive Medicare patient referralsAdvocate for patients needing coordinated, compassionate in‑home careCollaborate with clinical teams to ensure smooth transitions of careSupport outstanding patient outcomes and agency growthYou will represent Supportive Home Health Care with professionalism, empathy, and integrity while expanding our presence across Southern California.Key ResponsibilitiesCultivate and maintain strong partnerships with:Physicians & physician groupsHospitals & discharge plannersSkilled Nursing Facilities (SNFs)Case managers & community healthcare providersDrive timely and appropriate Medicare referralsEducate providers, patients, and families on home health services and eligibilityAssess patient needs and coordinate care plans with nursing and therapy teamsEnsure smooth transitions from hospital or facility to homeTrack referral activity, growth metrics, and territory performanceMaintain a consistent field presence to strengthen referral pipelines and expand market reachRequired QualificationsActive book of sales (mandatory)Minimum 2 years of recent experience as a Home Health Liaison or in home‑health‑specific marketing/referral developmentCurrent or very recent employment in a healthcare organization performing this roleDemonstrated Medicare referral successProven track record in healthcare sales, outreach, or referral growthStrong knowledge of Medicare home health eligibilityExceptional communication, relationship‑building, and presentation skillsExcellent time management, organization, and decision‑making abilitiesAbility to manage a territory independently and effectivelyIdeal CandidateEstablished referral relationships within hospitals, SNFs, and physician groupsHighly motivated, polished, and growth‑orientedStrong understanding of care coordination and patient transitionsProfessional, relationship‑focused, and passionate about improving patient outcomesSalary & Benefits$85,000-160,000 base salary (depending on experience)Performance bonusesFull benefits package, including:401(k)Health, Dental & Vision InsurancePaid Time OffFlexible ScheduleTravel ReimbursementImportant Application RequirementPlease do not submit a resume unless you have:An active book of sales, andAt least 2 years of recent Home Health Liaison or Medicare referral development experienceApplications missing these requirements will not be considered.
