This role is ideal for a licensed PT who is dedicated to improving patient mobility, independence, and overall quality of life through personalized, evidence-based therapy.Key ResponsibilitiesConduct comprehensive physical assessments and review medical histories to accurately evaluate patient conditions.Develop, implement, and adjust individualized treatment plans aligned with each patient's rehabilitation goals.Deliver therapeutic exercises and functional training programs to improve mobility, strength, and independence.Educate patients and their families on injury prevention, wellness strategies, and recovery expectations.Monitor, document, and evaluate patient progress, modifying care plans as clinically appropriate.Collaborate with a multidisciplinary healthcare team to ensure coordinated and comprehensive patient care.Maintain accurate and timely clinical documentation in compliance with regulatory and organizational standards.Required QualificationsGraduation from an accredited Physical Therapy program.Master's degree in Physical Therapy.Minimum of one (1) year of experience as a Physical Therapist in a home health setting.Active state licensure to practice as a Physical Therapist.Current CPR certification (required upon employment).Valid driver's license with the ability to travel to patient homes within the assigned service area.Strong interpersonal skills with a patient-centered, compassionate approach to care.Preferred QualificationsExperience working independently in a home-based clinical environment.Strong organizational and time-management skills.Compensation & Work ModelFull-time, field-based home health position.Competitive compensation package.Role requires regular travel to patient residences within the assigned geographic area.About the OrganizationOur client provides personalized home health services focused on restoring function, promoting independence, and improving quality of life.
