Medical Social Worker/Case Manager (Remote)

Overview The Medical Social Worker plays a vital role in providing support and resources to patients and their families as they navigate the complexities of healthcare. This position involves working closely with patients in various settings, including inpatient facilities and community care environments. The Medical Social Worker will assess patient needs, develop care plans, and facilitate access to necessary services while ensuring that emotional, social, and financial challenges are addressed. Responsibilities Conduct comprehensive patient assessments to identify psychosocial needs. Develop and implement individualized care plans in collaboration with healthcare teams. Provide counseling services, including child family counseling and PTSD care. Facilitate access to community resources and support services for patients and families. Document medical information accurately, ensuring compliance with coding and medical documentation standards. Engage in motivational interviewing techniques to empower patients in their care journey. Conduct research to stay informed about best practices in social work within a medical context. Collaborate with interdisciplinary teams to enhance patient outcomes in both clinic and inpatient settings. Skills Proficient in child family counseling techniques. Experience in home community care settings. Strong understanding of inpatient care dynamics. Skilled in motivational interviewing to effectively engage patients. Knowledge of coding practices related to medical documentation. Ability to maintain accurate medical documentation for patient records. Research skills to contribute to evidence-based practice improvements. Expertise in patient assessment methodologies. Familiarity with PTSD care approaches within a clinical framework.
