Ahava Therapy Services - PHYSICAL THERAPISTFull-Time, Part-Time, and PRN positions availableJoin Our Team!Ahava Therapy Services provides Physical, Occupational, and Speech Therapy across a variety of care settings.We are committed to delivering innovative, evidence-based care through teamwork and interdisciplinary collaboration.Our mission is to maximize each patient's potential while improving quality of life — leading the revolution in therapy services with compassion, respect, and clinical excellence.We hire therapists who share our vision and dedication, working diligently to restore function, prevent decline, and support patients in performing meaningful daily tasks.About the FacilityWhites Creek Wellness & Rehab is a 126-bed skilled nursing facility offering:Short-term rehabilitationLong-term careJoin a supportive in-house rehab team with strong leadership, dedicated to exceptional patient outcomes and meaningful collaboration.ResponsibilitiesReview resident information and evaluate its impact on assessment and treatment.Conduct and document thorough functional assessments using standardized procedures.Develop and implement individualized treatment plans with achievable goals.Comply with evaluation, treatment, and documentation guidelines; maintain timely, accurate records.Instruct and supervise therapy assistants as needed.Enter treatment and progress notes for each session.Revise treatment plans as needed throughout the course of care.Document treatment in clear, concise discharge summaries.Orient residents to services and treatment procedures appropriate to their needs.Collaborate with nursing staff, residents, and families to maximize functional abilities.Educate residents and caregivers on the continuation of home exercise and therapy.Assist with referrals for additional services as needed and maintain knowledge of community resources.Participate in rehabilitation team conferences and other facility meetings.Access continuing education opportunities to maintain clinical competency.Participate in quality improvement initiatives.Follow regulatory, professional, and facility standards.Perform other duties as assigned to provide quality care.Understand payer reimbursement and allocate therapy resources efficiently.Support Ahava's mission and project a professional image.Maintain expected productivity levels.QualificationsBachelor's or Master's degree in Physical TherapyCurrent PT license in the state of practiceBLS/CPR certification (current or obtained during orientation)Strong communication, teamwork, and organizational skillsBenefits (Full/Part-Time)Competitive Pay + Daily Pay OptionMedical, Dental, VisionShort/Long-Term DisabilityAncillary Insurance & FSAPTO + Free Birthday Holiday + Holiday PayFree Life Insurance & 401(k) with Company MatchFree CEUs and Continuing Education SupportBe part of something meaningful.Join Ahava Therapy Services — where compassion meets excellence, and therapists are empowered to make a difference every day.
