A Master of Science in Nursing or Nutrition, a Master of Public Health or a Master of Education is preferred.Experience For each level of education stated above, at least five (5) years of experience in the field of diabetes prevention and management or health administration is preferred.Licenses & Certification Licensed healthcare professional is preferred. Certification as a Diabetes Educator (CDE) or Certification as a Health Education Specialist (CHES) is preferred.Knowledge & Skills Knowledge of supervisory and managerial principles and skills including program planning and evaluation, directing program activities, funding and staffing in line with program goals.Knowledge of the principles and practices of health promotion/disease required in developing prevention and management plans.Knowledge of clinical and nutritional problems associated with diabetes.Knowledge of the appropriate education and training principles for all age groups as applied to public health practices for instruction of lay and professional groups.Skill in establishing realistic program goals and objectives, analyzing situations accurately and adopting and implementing effective courses of action.Ability to communicate courteously and effectively with patients and their families, medical and administrative staff, health care professionals and the general public on an individual and group basis, both orally and in writing.Ability to utilize a personal computer in a Windows environment.Ability to maintain a confident professional demeanor.Job SummaryThe purpose of the position is to coordinate the Diabetes Prevention and Management Program of the Muscogee (Creek) Nation Department of Health (MCNDH). Incumbent plays a vital role in the planning, designing, implementing, monitoring and evaluating of the Program. This is an exempt position as applicable to the FLSA.Work EnvironmentWork is performed in an office setting and in community settings at numerous facilities located within the designated service area. Driving a GSA vehicle is required.Physical DemandsWork requires the demands of normal office work, driving to communities and lifting of equipment and teaching aides used in performing position duties.Essential FunctionsSatisfactory job performance will be determined by successful execution of the following:Responsible for direction and management of the Diabetes Prevention and Management Program including planning, designing, establishing and evaluating the program; assessing community needs; converting plans to action by setting short-term/long-term objectives and sequencing activities.Responsible for preparing, writing and submitting grant applications and proposals to secure funding for program services and activities. Leading progress towards meeting programmatic activities, monitoring, evaluating and reporting outcomes to funder.Serves as spokesman for the MCNDH Diabetes Program with MCN Department of Health staff, MCN tribal citizens and leaders, community members, other tribes, state and national groups as appropriate to gather support and understanding of program services and to share learning.Has oversight responsibility for maintaining Accreditation of Diabetes Education Services by preparing, writing, submitting applications and maintaining and/or updating curriculum and policies.Prepares, justifies and administers the Diabetes Prevention and Management Program budget/budgets; approves and tracks expenditures. Assures the availability of adequate supplies, equipment and facilities; monitors overall status of activities in the work unit identifying problem areas and taking corrective actions.Assures that all required reports and data are provided to the Indian Health Service (IHS), MCNDH leadership, and facilities/programs in a timely manner and assures that the program is in compliance with any standards being promulgated by ADA, IHS and the Muscogee (Creek) Nation.Oversees and coordinates workgroups and committees in planning activities, services and programs provided through Diabetes Prevention and Management Program, providing guidance, identify processes and technical expertise in program direction to ensure activities, services and programs are run smoothly, efficiently and meet deadlines or expectations.Assures broad community and professional input into program activities using formal and informal activities such as Diabetes Advisory Committee, surveys and focus groups located throughout the Muscogee (Creek) Nation.Responsible for establishing and updating current, appropriate policies and procedures covering the activities of the Diabetes Prevention and Management Program.Collaborates with the Diabetes Program educators and clinical staff in developing curricula, updating curriculum, implementing and selecting how to measure success for participants and program.Collaborates with Health Administrators and Clinical Directors to promote adherence to standards in diabetes care.Communicate with MCNDH through meetings and reports.Coordinates grant and program evaluation.Monitors licensure of Diabetes Program staff.Responsible to initiate and set goals for Diabetes program based on the organization's strategic objectives.Responsible to adequately staff Diabetes Program needs at all clinical locations ensuring daily tasks are completed including monitoring schedules to ensure patient needs are meet in timely fashion while providing support to assure patient success.
