This role is essential in ensuring accurate assessments, regulatory compliance, and optimized reimbursement. The accuracy of this work directly influences the facility’s CMS 5‑Star rating and quality measures.Key ResponsibilitiesLead the MDS ProcessDetermine optimal ARD (Assessment Reference Dates)Ensure timely completion of all assessmentsMonitor and audit MDS accuracyAssessment & DocumentationComplete MDS assessments for short‑stay patients and long‑term residentsConduct chart reviews and prepare appeal submissions for insurance auditsTrack Priority Health dashboard data and outcomesIssue NOMNC and ABN forms to patientsInsurance & Audit SupportComplete insurance certifications and re‑certificationsProcess MPRO and KePRO audits and appealsCare Planning & CollaborationAttend care conferences and discharge planning meetingsAssess resident physical and psychosocial needsIdentify resident problems and expected outcomesDevelop and update individualized care plansIncorporate resident/family input and interdisciplinary team dataImplementation & EvaluationImplement evidence‑based interventionsAdminister medications and treatments per approved nursing practicesMaintain resident comfort and safetyProvide resident education and coordinate care plansEvaluate resident progress and document outcomesAdditional ResponsibilitiesParticipate in nursing stand‑by coverage; may serve as charge nursePerform other duties as assignedQualificationsAssociate Degree, Diploma, or BSN from an accredited nursing programLicensed Registered Nurse in good standing with the State of MichiganMDS experience preferredExperience in long‑term care, rehab, or geriatrics preferredMDS certification required within one year of hireBLS certification required by completion of core orientationSkills & CompetenciesExcellent communication, interpersonal, and organizational skillsStrong critical thinking and prioritization abilitiesHigh level of accountability and responsibilityAbility to seek information from multiple sourcesProficiency with word processing, spreadsheets, and presentation softwareHighly organized with strong accuracy and attention to detailPhysical & Work RequirementsFrequent mental and visual focus with tight deadlinesComputer use up to 40 percent of the timeAbility to regularly lift up to 100 pounds and assist with heavier tasksWork may require reaching, climbing, balancing, and handling heavy objects
